Ordering

Complete the following steps to submit an order:

1. Use Online Order Form or click here to download the Reemo Referral form.
   Please note: Download the most current version of the referral form from this site for future orders.

2. Complete the Reemo Referral form (including all of the required fields) with the individual’s information.

3. Email the document as an attachment in .xlsx file format (not PDF) to orders@reemohealth.com.

The watch will ship within five business days and if emails were provided, a shipping notification will be sent to the individual and the Case Manager.
